What causes lower back pain and sciatica?
Back problems are extremely common, especially with long hours of sitting or physical work. The usual causes include:
- Muscular and postural strain — the most common cause, often from sitting, lifting or poor posture.
- Slipped (prolapsed) disc — where a disc presses on a nerve and can cause sciatica.
- Sciatica — pain that travels from the back into the buttock and down the leg, sometimes with numbness or tingling.
- Age-related changes — such as wear in the spine (spondylosis), more common with age.
Is it sciatica?
Sciatica typically causes pain that runs below the knee, and may come with pins and needles, numbness or a burning feeling in the leg. It often feels worse when sitting for long periods, or when you cough or sneeze. The good news is that most sciatica settles with the right guidance and does not need an operation.
How an online consultation with Prof. Dr. Ahmed Ali helps
Back pain is diagnosed mainly from the pattern of your symptoms and your history — which works very well on a video call. Dr. Ahmed will listen carefully, guide you on posture, movement and physiotherapy, prescribe relief where needed, and tell you clearly whether an MRI is actually required. You leave with a written plan and a proper understanding of what is going on.
Will I need surgery or an MRI?
Usually not. A slipped disc on a scan does not automatically mean surgery — many people walking around pain-free have disc bulges on imaging. Prof. Dr. Ahmed Ali treats the patient, not just the picture, and will give you an honest opinion on whether a scan or a surgical opinion is genuinely needed. If you have already been told to operate, an independent second opinion is often worthwhile before deciding.

How do I know if my back pain is sciatica?
Sciatica is pain that travels from the lower back into the buttock and down the leg, often below the knee, and may come with numbness, tingling or a burning feeling. It is usually caused by a slipped disc or pressure on a nerve. A consultation can confirm whether your pain fits this pattern.
Does a slipped disc always need surgery?
No. Most cases settle without surgery using the right guidance on posture, movement, physiotherapy and medicines. Surgery is only considered in specific situations such as progressive weakness. Dr. Ahmed will tell you honestly which category you are in.
Do I need an MRI for back pain?
Not always. Many people are managed without an MRI. Dr. Ahmed will assess your symptoms and tell you whether a scan is genuinely required, so you do not pay for one you do not need.
Can back pain and sciatica be treated online?
Yes, for the large majority of cases. The diagnosis comes mainly from your history and the pattern of pain, which Dr. Ahmed reviews carefully on video, along with any reports you have.
How much does a back pain consultation cost?
Rs 5,000 per session — including the video consultation, review of any scans, a diagnosis and treatment plan, and a prescription where appropriate.
